Journalist Sarah Macdonald, in her twenties, left India with a lingering sense of poverty, pollution, and heat. She cried out, "Never!" and gave India and the beggar the finger when he read her palm and informed her she would go back—and for love.

However, the prophesy comes to pass eleven years later. Sarah leaves her perfect career to relocate to New Delhi, the world's most polluted city, when her true love was assigned to India. This looks to Sarah to be the ultimate act of loving sacrifice, and it almost kills her. After settling in, she has double pneumonia, which puts her in danger and forces her to confront her own frail mortality and inner spiritual emptiness. She says, "I have to find peace in the only place in India that is possible." “Inside.” Her quest to learn the meaning of life and death via India thus begins.

Holy Cow is Macdonald's often amusing account of her travels through a chaotic and contradictory nation, where she met adherents of several religions, including Hinduism, Islam, and Jainism; Sufis, Sikhs, Parsis, and Christianity; and a colorful cast of Bollywood celebrities, yogis, and swamis. A lady on a quest to rescue her soul, her love life, and her sanity can only make it via this voyage, which takes her from spiritual retreats and decaying nirvanas to conflict zones and New Delhi nightclubs.
